Nose Surgery

The nose is the center of the face, a part of us that is simply
impossible to hide. An unattractive nose basically captures all the
attention from even the most attractive face. Each year thousands of
people undergo cosmetic nose surgeries. An unattractive nose could
be due to hereditary factors, an injury or simply aging factors. The
nose could be wide from either the bony part (where we rest our
glasses), or the caliginous part where we (hold to blow our nose). It may also be too
drooping or have a hump that is mostly prominent from the profile
view. The nasal septum (divider between the two sides of the nose)
may be deviated causing the nose to look crooked and impairing
breathing as well. A nose surgery can correct all that making the
nose both attractive and properly functional.

What is a Nose Surgery?
Nasal
Surgery or "Rhinoplasty"
is a surgical procedure where the
shape and size of the nose is refined while maintaining its
function. Multiple correction techniques for the nasal bone and
cartilage are used depending on your case, your desired changes and
Dr. Metwally’s recommendation.

Are You a Good Candidate for a Nose Surgery?
You may feel uncomfortable about how your nose looks from the front
view or from the profile view or both. The bony part of your nose
may be too wide, or you may have a hump (mostly noticeable from the
profile), you may have a wide tip (the lower end of the
cartilaginous part) or a droopy long nose. Your septum may be
deviated. Your nostrils may be too wide or your nasal tip is too
close to your face, missing the proper tip support to give you the
attractive elevated nasal tip. The nose surgery can be performed
starting age 18.
Preparing for Your Surgery
Before your surgery, Dr. Metwally will perform a complete evaluation
with you. Factors like your facial proportion, skin type, bone,
cartilage, inner lining or ethnic appearance are all discussed
during the evaluation. It is very important to understand that the
same procedure, done in the exact same way on 2 different noses may
give very different results according to the factors mentioned
above. Dr. Metwally will explain these factors to you thoroughly,
understand your desired goals and explain the expected to and from our center the day of the surgery.
You
will need to arrange for someone to drive you to and from our center
the day of the surgery.
You
should wear loose fitting clothes and bear in mind that your outfit
may get soiled. Do not wear jewelry, contact lenses, or bring
valuables to the operating room. Remove all body jewelry from
pierced body parts.

Duration of the Surgery
It takes around 1 hour to 2 hours for Dr. Metwally to perform a nose
surgery depending on the level of correction made.
Anesthesia Used
The
surgery can be performed under
local anesthesia with sedation (you won’t be totally asleep but you
will be relaxed and drowsy and won’t feel anything during the
procedure). It can also be performed under general anesthesia (you
will be asleep throughout the procedure). You will be ready to leave after 1 to 2 hours from the
completion of your surgery depending on the anesthesia used.

During Your Surgery
Two main incision techniques are used in the nose surgery. One is
called a “Closed Rhinoplasty”, and the other is called an “Open
Rhinoplasty”. In the "Closed Rhinoplasty", all incisions made are on
the inside of the nose. In the "Open Rhinoplasty", a small part of
the incision approximately 3 mm is made across the columella (the
long part between the 2 nostrils). If you wish to have your nostrils
smaller you will have a tiny incision 2-3 mm on the outer side of
each nostril hidden in the crease between the outer side of the
nostril and the cheek. Dr. Metwally will choose the technique that
best suits the level of correction you desire in your nose. After
the incision is made, all the remodeling of the bone or cartilage is
done and a few sutures are made. In most of the cases where
remodeling of the bone is made you will have a cast on your nose for
approximately 7 days.

After Your Surgery
After your surgery you will find that you have some bruising and
swelling around your eyes, this is normal, and subsides from 7 to 10
days. Very little discomfort may be present for the first 2 days,
Dr. Metwally will prescribe to you an antibiotic to prevent
infection and a pain killer to minimize the discomfort. Some
discoloration may be present this can be covered by concealer and
subsides within 10 days. Most people are ready to return to work
after 8 to 10 days from the surgery. You can shower normally from
the second
day while watching out not to wet your nose. You can wet your nose
once the cast is removed. You can wear your glasses on top of the
cast and after the cast is removed, but be careful not to hit your
nasal bone during the first month of the surgery. You should refrain
from vigorous exercise and horse-back riding for 1 month after the
surgery.

Seeing Your Result
How soon the result starts to show often varies from one person to
the other depending on many factors. Some people heal faster than
others for reasons like age, skin color and skin type. Other factors
are related to the level of correction made in the surgery like the
amount of bone and cartilage removed and remodeled, and the
surgeon’s skills. According to Dr. Metwally’s observation of his own
patients, they all start seeing results as early as the 7th day of
the surgery (upon cast removal) and the final result usually takes 8
to 12 weeks. After that very minor changes in the appearance of your
nose will occur until 6 months of the surgery have passed when
complete healing is reached. The tiny scar in the columella starts
fading 7 days after the surgery and is hardly ever visible after 4
to 8 weeks.
