Breast Augmentation

Female breasts are every woman's most precious asset. It's the part of us that reveals most of our femininity. Women may choose to undergo a breast augmentation surgery for various reasons. The breasts may be under developed, they may have lost size, shape and firmness after pregnancy or breast feeding. They may have lost fullness after weight loss or simply with age. Breast augmentation surgery can correct the breast shape and size and is one of the most commonly performed procedures incosmetic plastic surgery. Breast enhancement using implants can give women a more proportional body thereby tremendously improving self esteem.

What is a Breast Augmentation Surgery?

Breast Augmentation or "Augmentation Mammoplasty", is a surgical procedure designed to increase the size and enhance the shape of female breasts. It can also correct breast asymmetry and volume loss after pregnancy and lactation. Some women seek breast augmentation to improve the proportion of their bodies, since most bodies tend to be fuller at the lower half. A lot of Dr. Metwally’s patients report looking thinner on their lower body after enlarging their bust line.

Are You a Good Candidate for Breast Augmentation?

You would be a good candidate for breast augmentation if you have small breasts, have lost a significant amount of weight that has caused your breasts to lose volume, have lost your perky breasts after pregnancy and or breast feeding or have breasts that are not proportional in size with your fuller lower body. In some cases, if your breasts are droopy you may need a breast lift along with the breast augmentation procedure to achieve best results. Breast augmentation can be performed starting age 18.

Preparing for Your Surgery

Before your surgery Dr. Metwally will perform a complete evaluation with you. During this evaluation the type, size and placement of implants most suitable for you will be decided upon. Implants are either round or teardrop in shape. Dr. Metwally mostly uses the round shape since it eliminates any risk of distortion if implants were to flip or rotate. Teardrop shaped implants are mainly used in reconstruction since the implant will have more weight on its lower part versus its upper part. Implants also come smooth or textured. Dr. Metwally almost always uses the textured implants since they allow better tissue adherence with the implant. The external shell is made of a strong silicone elastomer shell. The inner content of the shell differs; it could be Para-gel (Silicone + hydro-gel), Hydro-gel (water based gel), soy oil gel or saline. The incision is made either in the infra-mammary fold (breast crease), Transaxillary (armpit), Areola (the dark circle around the nipple), or TUBA / Transambillical (belly button). One incision may be better than the other for you depending on the typeof implant you’re using, the desired size and the placement of the implant. Implants are placed either subglandular (beneath the gland) or submuscular (beneath the muscle). Dr. Metwally almost always uses the submuscular placement since the implant istotally covered by the musclewhich hides the implant edges.

It also reduces the risk for capsular contracture (where the breasts form a hard capsule around the implant) and gives better mammogram readings. All these factors will be discussed prior to your surgery, and Dr. Metwally will explain to you the procedure that best serves your case. You will need to arrange for someone to drive you to and from our center the day of the surgery. You should wear loose fitting clothes and bear in mind that your outfit may get soiled. Do not wear jewelry, contact lenses, or bring valuables to the operating room. Removeall body jewelry from pierced body parts.

Duration of the Surgery

It takes around 30 minutes to 45 minutes for Dr. Metwally to perform a breast augmentation surgery depending on the technique used and the placement of the implant.

Anesthesia Used

The surgery is usually performed under general anesthesia (you will be asleep throughout the procedure). You will be ready to leave after 1 to 2 hours from the completion of your surgery.

During Your Surgery

During your surgery an incision will be made in one of the locations mentioned above after which a pocket is created to accommodate the implant. The pocket is made according to the implant placement as described earlier, behind the gland or the muscle. The implant is then inserted; centered behind the nipple and the incision is sutured. Mild dressing will be applied to your stitches and a skin color bandage will cover your breasts for a few days providing support to your breasts to encourage healing.

After Your Surgery

You will experience some pain during the first 48 to 72 hours. Dr. Metwally will prescribe to you an antibiotic to prevent infection and a pain killer to alleviate pain. Most of Dr. Metwally’s patients report feeling pressure in their breasts for the first 1 to 2 weeks. The bandage covering your breasts will be removed 5 days after surgery. After that you will be asked to wear a special bra for 1 month that supports your breasts while they heal. Your stitches will be removed 2 weeks after surgery. Most people feel good enough to go to work 3 to 5 days after their surgery is performed. You may shower one week after surgery. Immediately after your surgery, your breasts will appear a bit too firm and high. This is normal due to the swelling. The swelling will subside after 4 to 6 weeks and your breasts will settle in their natural position and feel within 4 to 6 months at the most. Your breasts may be sensitive to direct contact for 3 to 4 weeks. You should avoid strenuous activities, vigorous exercise and sleeping on your breasts for 1 month.

Seeing Your Result

The beauty of Breast Augmentation is that you can see your initial result immediately after you finish your surgery. However, as mentioned above your breasts will be extra firm and a bit too high, but the size is more or less what you see right then. After that your breasts will swell making them appear larger than they actually will be when the swelling is gone. The final result varies from one person to another depending on many factors. Some people heal faster than others for reasons like age, implant placement and of course the surgeon’s skills. According to Dr. Metwally’s observation of his own patients, most of them see their final results after 12 to 16 weeks. The scar from the incision takes from 12 to 18 months to fade totally.
